Substance Abuse Assessments
The purpose of an assessment is to evaluate your relationship to substances and make recommendations based upon the results.
Various instruments are used to help the counselor conducting the assessment determine how your relationship to substances is affecting your life. These instruments are questionnaires that you will fill out when you arrive at our office. After filling out the questionnaires you will meet with the counselor who will interview you and ask you more questions which at times may seem repetitive. All of this information will then be scored and evaluated by the counselor who will contact you in 1-3 days with their findings.
It is important that you are honest throughout the assessment. Do not answer questions the way you think they should be answered, just answer all questions honestly.
Many people must participate in an assessment for legal reasons. Our assessments have been accepted in many court systems and motor vehicle departments in most states. You should bring whatever court papers you have so that we can determine if one of our counselors meets the credential requirement of the entity asking for the assessment and if there are any instructions from the entity that requires the assessment. We can refer you to an outside professional in the event that we are not appropriately credentialed to perform the assessment
Assessments cost between $200-$350.
We perform assessments for an individual’s own use or for criminal court, probation, employers, EAP’s and schools.
We do not perform assessments for civil proceedings, Family Court (divorce or custody cases) or to “prove” to a significant other that a problem does/does not exist.
We cannot assess a person for mental health issues but would be happy to refer you to someone who can.
Please call 415-296-9900 to make an assessment appointment.
