If you have already made an intake appointment over the phone, please read the following important information before coming to your appointment:
- You may sign up and attend an intake appointment before going to court.
- If you have gone to court and have been sentenced, make sure that you have scheduled an intake appointment within the time-frame specified by the court.
- Arrive for your intake appointment on time. Late arrivals will not be accommodated, and you will need to reschedule.
- Be free of alcohol and drugs (excluding prescribed medications) when you arrive for your intake appointment. We recommend that you abstain from alcohol and drug use for 24 hours prior to your intake appointment. If we suspect that you are under the influence, we will breathalize you and, if you test positive, you will be turned away.
- Be absolutely certain to bring your court papers.
- If you have not gone to court or if the court is not involved with your case, bring either a DMV printout or the pink slip of paper (Admin Per Se) given to you on the date of the arrest. The Admin Per Se is served as a temporary license for 30 days following your arrest.
- If you are transferring from another DUI program, bring your transfer papers and be within the 21 days allowed to complete the transfer.
- Know your arrest date and blood alcohol level at the time of the arrest.
